Fix for possibility of race condition in point-to-point MPI
communications.

Fixes for warnings produced by clang.

XML format now can use Include attribute in any XML tag to address
external files.

Added mechanism to name meshes and retrive meshes by name.

Added feature to store data of RemoteHandle type that allows to address
elements on another mesh. Loading this data type from files is still
awkward.

broken conformity of the grid during modification, more attention to the

Fixed issues that files overwritten with MPI_File* functionality may
contain previous information.

Adapting mesh_file.cpp to new comparator's behavior.

Zoltan partitioner now works. Please refer to wiki for compilation with
Zoltan on windows.

Works on windows with parmetis-4.0.3. Added feature:  INMOST can provide
memory usage per element to ParMETIS.

Huge update involving update of underlying data representation and
elements management. Different interaction with iterators. New
ElementSet representation.  Some annotation for documentation is added.
New data structures. Examples require update. (check
examples/CMakeLists.txt)

Fixed issues in GridGen that prevented compilation by microsoft visual
studio compiler;
Fixed small bug introduced by previous commit.

Moving connections, markers, geometric type, sparse data from Storage
class to TagManager class by using standard handling of mesh data.

Retrieval of elements by markers;
Introduced options for loading of files;
Fixed issues saving vtk format when not all topological tests are
active;
Introduced Connect / Disconnect functions for manipulations with mesh;
DrawGrid example can now visualize array of entered cells/faces for
debug purposes;
OldDrawGrid example demonstrates fast interactive mesh slicing and
onscreen element picking.
